Based on what you've described, service would be needed.
If you'd like to check the Picture Test in Device Care, here's the basic steps:
Settings > Support > Device Care.
For service, please feel free to reach out to Customer Support at 1-800-Samsung (1-800-726-7864), Online at https://www.samsung.com/us/support/service/, or sending a private message to any moderator.
Please have this information handy when reaching out: Model Number, Serial Number, First and Last Name, Best Contact phone number, Address where the unit is located, and Email address.
To help the service process, we would recommend taking photos of the screen showing the Picture Test Image(s) and/or a photo or two showing the TV on a normal channel/app showing the lines.
